Rowans Hospice hosts the Palliative Care Support Hub in partnership with Hampshire and Isle of Wight Healthcare NHS Foundation Trust (previously Southern Health NHS Foundation Trust and Solent NHS Trust) and Portsmouth Hospitals University Trust.

Patients, carers or health professionals can call the Hub on 023 9225 0001 option 1 between 9:00am – 5:00pm Monday to Friday.  Here you will speak to a specialist Clinician or Administrator who will be able to offer support and advice to help coordinate your care.  Outside of these hours advice and support is available to healthcare professionals.

We work with local care providers, GPs, district nurses, Macmillan nurses, specialist nurses and other health and social care professionals to coordinate and provide the best possible care.

Who can call the Palliative Care Support Hub?

We welcome calls from anyone seeking information or advice about Palliative or End of life care in Portsmouth & South East Hampshire.

Who will I speak to?

You will always be able to speak to a Clinician or an experienced Clinical Administrator.​

When should I call?

We are available 24/7. We would encourage you to call during normal working hours (9:00am – 5:00pm weekdays) if you can, and as early as possible in order to give us the best chance of being able to support you, particularly if we need to refer you on to other services.​

Will I get help or will you just pass me onto someone else?

With your consent, we will access information, in order to provide the best advice for you. We aim to provide a plan for the help you require, or to signpost you quickly to the most helpful services. We will often make that referral on your behalf in order to help you access appropriate support. We will work very closely with other services on your behalf. ​

Will they tell my GP I called or do I need to?

Together we will decide if we or you are best placed to contact your GP.

Can I speak to my OT/Physio/Nurse/Doctor specialist?

Yes, with your consent can link with any specialist who is involved in your care as needed.​
