Assessment by a healthcare professional is required before a hospice referral for admission to our Specialist In-Patient Unit can be considered.
If you are an HCP and would like to refer someone, please contact a senior clinician via the Palliative Care Support Hub on 023 9225 0001 (option 1) during office hours (Monday – Friday, 9am – 5pm).
