To make a referral to the inpatient unit, Hospice at Home service or our Community Palliative Care Team partners, please contact the the Palliative Care Support Hub on 023 9225 0001 (option 1) during office hours (Monday – Friday, 9am – 5pm).
A senior clinician will discuss your referral and can email you the most appropriate referral form if needed.
