Patients and their families are at the centre of everything we do. We listen to their concerns and respond to issues causing anxiety by providing the resources and assistance they need. We offer impartial guidance to help them make informed choices about their care and well-being.
The Social Work team liaise closely with other teams in the Hospice, for example the Ward Staff, Day Care, Psychology Services, Rowans Meerkat Service and Community Care, to provide a wide range of practical and emotional support.
How might a Social Worker help?
Our Social Workers are available to help in many different ways, for example if you are admitted to the inpatient ward, you may be worried about how you will manage on discharge. We can meet with you, your family or carer to ensure that you are fully involved in planning your care and to enable you to consider all options of support for when you return home. These options may involve returning home with a package of care, or alternatively, moving to a nursing home setting. A Social Worker will be able to support and guide you and your family or carer through the choices available to you.
If you are attending Day Care Services at Rowans Hospice, you or your family may need support or advice about your care in order to maintain your independence. We can meet with you at the Hospice or visit you at home to discuss your options and develop a package of care for you.
Social Workers can also be involved in providing:
- Carer support
- Financial and/or benefits advice
- Liaising with the Local Authority Social Services Department
- Signposting to other services or agencies.
We can also advocate on your behalf, helping you to communicate with other healthcare professionals to ensure you understand exactly what is happening and ensure your preferences are heard and understood.
Who can access a Social Worker?
A Social Worker is available to all patients, carers and families receiving a service from Rowans Hospice. If you are not known to Rowans Hospice, or are unsure whether we can help, please contact us.
