Patients and their families are at the centre of everything we do. We listen to your concerns and respond to issues causing anxiety by providing the resources and impartial assistance you need  to help you make informed choices about your care and well-being.

How might a Social Worker help?

Our Social Workers help in many different ways, offering advice, support and resources to help you manage your care.  They liaise closely with other teams in the Hospice to provide a wide range of practical and emotional support.

For example, if you are admitted to the inpatient ward and are worried about how you'll manage upon discharge, we'll meet with you, your family or carer to ensure you are fully involved in planning your care and make you aware of options of support for when you return home.

These options may involve returning home with a package of care, or moving to a nursing home setting. We will guide you and your family or carer through the choices available to you.

Social Workers are also involved in providing:

  • Care packages to help you maintain your independence
  • Visits at the Hospice or in your own home to discuss your care needs
  • Carer support
  • Financial and/or benefits advice
  • Liaising with the Local Authority Social Services Department
  • Signposting to other services or agencies.

We can also advocate on your behalf, helping you to communicate with other healthcare professionals to ensure you understand exactly what is happening and ensure your preferences are heard and understood.

Who can access a Social Worker?

A Social Worker is available to all patients, carers and families receiving a service from Rowans Hospice. If you are not known to Rowans Hospice, or are unsure whether we can help, please contact us.
