Lead Social Worker

Band 7 (£38,890 – £44,503 FTE, dependant on experience)

30 hours per week

The Rowans Hospice Charity is an independent organisation offering specialist palliative care to patients with life limiting disease, located in Waterlooville, Hampshire.

As a Charity, we offer opportunities for palliative support to patients and their families over an extensive range of in-patient and out-patient services; we pride ourselves on our excellent care for patients, their families, our staff and volunteers. The Hospice is proud to have been rated as outstanding in the latest CQC inspection.

An opportunity has arisen to join the Charity as a Lead Social Worker. The role is responsible for the management of the social work team, along with the provision of social work/care management service to patients and carers in facilitating discharge home, or a smooth transfer to an alternative care environment.

The successful applicant will have the required qualifications in Social work and be able to demonstrate relevant experience in care management and strong counselling skills. We are looking for an individual who has a keen interest in working with families and an awareness of bereavement issues. They will need the ability to participate in the development of services within a multi-professional team. Teaching skills, leadership skills and evidence of continuous career development is essential.

Main accountabilities:

  • To provide a flexible range of services to patients referred to the Rowans Hospice Charity and to their families and carers. Such services will include psychosocial support for individuals, their families and groups, delegating responsibilities where appropriate.
  • To ensure provision of a care management service for patients referred to the Hospice and their families.
  • To allocate referrals and co-ordinate assessments, produce care plans and arranging and secure funding for appropriate packages of care.
  • To work as part of a multi-disciplinary team in the overall co-ordination of services to patients.
  • To provide liaison between the Charity, the Department of Social Services and other agencies and organisations, including local Primary Healthcare Trusts.
  • To participate in monthly relatives’ evenings, assist with group work and family work including service development in liaison with Bereavement Service Co-ordinator.
  • Adult Safe Guarding Lead for the Charity
