Band 7 (£38,890 – £44,503 FTE, dependant on experience)

Clinical On-Call Supplement

37.5 hours per week

The Rowans Hospice Charity is an independent organisation offering specialist palliative care to patients with life limiting disease, located in Waterlooville, Hampshire.

As a Charity, we offer opportunities for palliative support to patients and their families over an extensive range of in-patient and out-patient services; we pride ourselves on our excellent care for patients, their families, our staff and volunteers. The Hospice is proud to have been rated as outstanding in the latest CQC inspection.

An opportunity has arisen to join the Charity as a Hospice at Home Team Manager. The post holder will be responsible for leading our well-established Hospice at Home team in delivering a high level of palliative care to patients within their own homes. We are looking for a self-motivated and driven individual, who will uphold our commitment to outstanding personalised care to our patients and their families.

The successful candidate will be able to demonstrate sound clinical knowledge of end of life medications and the management of providing care at home for patients with a life limiting illness. We are seeking a forward-thinking individual who will be passionate about our patients and be committed to ensuring the growth and stability of the team continues. Experience within specialist/palliative care and demonstrated managerial experience is essential.

Main accountabilities:

  • Ensuring that the highest standards of holistic nursing care are provided through careful skilful organisation, while maintaining responsiveness to new referrals and altering care needs within the caseload.
  • To co-ordinate, develop and manage the Hospice at Home service in line with the Rowans strategic and operational plan.
  • To represent the Hospice at Home at Multi-Disciplinary Team (MDT) meetings.
  • Develop a culture of continuous improvement, pro-actively seeking feedback and identifying opportunities to advance and develop.

We are looking for the following knowledge, skills and abilities:

  • RGN or RN Adult Branch.
  • On NMC Register parts 1 or 12
  • ENB 931/285/237/998 or equivalent/or relevant area of study
  • 5 years’ experience in Palliative Care– that equates to Degree Level

or

  • 5 years’ experience in community care at D/N level – that equates to degree Level
  • 3 years proven leadership/management experience
  • Evidence of service development and implementation
  • Driving License/Car Owner
