Rowans Band 6 – £32,305 – £39, 026

30 hours   per annum FTE

Fixed Rota within Hospice at Home, including alt weekends

Rowans Hospice is an independent Charity offering specialist palliative care to patients with life limiting disease. We are based near Portsmouth on the south coast. Rowans Hospice offers opportunities for support to patients and their families over extensive range of in-patient and out-patient services; we pride ourselves on our excellent care for patients, their families our staff and our volunteers. The hospice has been rated as outstanding in the latest CQC inspection and we are looking for someone to join our committed team within Hospice @ Home who will uphold our commitment to outstanding personalised care.

The Rowans Hospice at Home team offer end of life care in the community, working closely with NHS Primary Care Services. We provide support to patients, family members and carers, especially when there is intense need – be that physical, psychological or spiritual.

An opportunity has arisen for a confident, enthusiastic and highly motivated clinician, who can work as a Deputy Hospice at Home Manager. The management structure comprises of a Hospice at Home Manager and Deputy Hospice at Home Manager. We are seeking someone who has experience in specialist/palliative care, a professional background of at least five years post qualification and some managerial experience of at least a two years at Band 6. The successful candidate will be able to demonstrate sound clinical knowledge of end of life medications and management of the care at home of patients with a life limiting illness. The Hospice at Home service has continuously developed since its inception. It does not stand still in its efforts to meet patients changing needs and to adapt to the complex climate of care that the community presents. Any successful candidate will be a forward-thinking individual who is passionate both about people cared for by the team and committed to ensuring the growth and stability of the service. We need someone who is dedicated to providing excellent, individualised end of life care.

Main accountabilities:

  • Manage and triage the Hospice at Home caseload, ensuring that the highest standards of holistic nursing care are provided through careful, skilful organisation. Maintaining responsiveness to the assessment of new referrals and the changing care needs of existing patients.
  • To co-ordinate, develop and manage the Hospice at Home service in line with the Rowans strategic and operational plan.
  • To represent the Hospice at Home at Multi-Disciplinary Team (MDT) meetings
  • Develop a culture of continuous improvement, pro-actively seeking feedback and identifying opportunities to advance and develop. Participate in managerial and clinical supervision.
  • To demonstrate commitment to continuous professional development for yourself and others.

We are looking for the following knowledge, skills and abilities:

  • At least three years post NMC registration experience
  • Solid knowledge and experience within a palliative care setting.
  • Diploma/Graduate or equivalent working experience.
  • Success through working with a multi-disciplinary team.
  • Excellent interpersonal and influencing skills and someone who can quickly establish credibility and build strong working relationships.
  • The ability to prioritise activities and multi-task to meet goals and deadlines.
  • The willingness to work collaboratively, as part of a team – but also a flair for working independently and being accountable for the quality of personal outputs and success.
