Giving choice when it matters

We know that most people in their final days of life would choose to die peacefully and with dignity in their own home, among those who are important to them. However, the fear of distressing symptoms and worries about the physical and emotional demands being placed on carers, relatives or friends, often results in people going into hospital to die.

The Rowans Hospice at Home service aims to offer additional support at this cruicial time in people's lives to enable them to stay at home and receive high-quality end of life care.

The service is available 365 days a year and offers patients and carers access to specialist advice and nursing visits on a 24 hour basis.

What is Hospice at Home?

The service aims to deliver high-quality nursing care that is routinely provided at The Rowans Hospice to patients and their families within their own homes.

This includes:

• The provision of dedicated and holistic end of life care for those who wish to die at home
• Specialist advice on symptom control and the management of distress to include spiritual and psychological support
• The promotion of dignity and peace for those who are dying
• The provision of planned, supportive visits to enable carers to continue caring, or to enable those living alone to be supported at home
