Area of care

Our area of care is determined by the patients of the GP practices we serve. For this reason the boundary line is not fixed and so the map is only a guide.

The doctors, nurses and clinical psychologists in the Community Care team provide high quality specialist palliative care at home, in nursing homes and in hospitals. Our spiritual care, Family Services and Bereavement Services also extend beyond the hospice to patients and families across Portsmouth and south east Hampshire.
